In the Home
The family remains the primary caregiver for home patients. Our hospice care team provides education, support, and expert medical care to the patient. Phone support and visits are available around the clock to address symptoms and to facilitate communication with the patient’s care team.
In the Nursing Facility
Hospice services do not take the place of care provided by the nursing home staff. JACH provides additional layer of care. Our team works with the facility to coordinate the best care possible.
In an Assisted Living Residence
Our Hospice team provides the care and support needed during the final months
of life. Care is coordinated for each patient to assure they receive “just what they need” for comfort and care.
In a Hospital
Patients can receive hospice care while in the hospital. JACH will work side by side with the hospital staff to ensure comfort and dignity. This is generally a short term stay until the patient’s pain and symptoms are under control and can be moved to wherever “home” is located.
In the Hospice Home
Critical patients needing short-term symptom management receive individualized and specialized care in the comfortable, tranquil environment at the JACH Hospice Home located in Joliet. Family members are welcome to stay with the patient and care is 24 hours day.
No matter the setting, the hospice care team of nurses, aides, doctors, social workers, chaplains and volunteers see that every measure of comfort is provided to the patient, including communications with the family and primary physician as needed.
